There were no supports beneath the Youngman boards he fell on, causing him to plummet four-and-a-half meters to the concrete floor below.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Mr Ife fractured his skull and damaged his facial nerves, leading to a three-day stay in hospital.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The Health and Safety Executive (HSE) launched an investigation and found several shortcomings on the part of 2 Counties Construction (Midlands) Ltd.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<h3>2 Counties Construction (Midlands) Ltd were found guilty of multiple shortcomings<\/h3>\n<p><span data-contrast=\"auto\">These shortcomings included a lack of appropriate measures to prevent falls into the building and stairwell openings.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Issues with inadequate planning, lack of proper equipment selection for <a href=\"https:\/\/www.pbctoday.co.uk\/news\/health-safety-news\/how-implement-safer-working-at-height\/119948\/\" target=\"_blank\" rel=\"noopener\">working at height<\/a>, and insufficient site management arrangements were also found.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">2 Counties Construction (Midlands) Ltd admitted to violating <a href=\"https:\/\/www.legislation.gov.uk\/uksi\/2015\/51\/regulation\/13\/made#:~:text=13.,risks%20to%20health%20or%20safety.\" target=\"_blank\" rel=\"noopener\">Regulation 13(1)<\/a> of the Construction (Design and Management) Regulations 2015.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">This regulation states the following: \u2018<\/span><span data-contrast=\"none\">The principal contractor must plan, manage and monitor the construction phase and coordinate matters relating to health and safety during the construction phase to ensure that, so far as is reasonably practicable, construction work is carried out without risks to health or safety.\u2019<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The company received a fine of \u00a312,000 and was instructed to cover \u00a34,139 in costs by the Cannock Magistrates\u2019 Court on 21 July 2023.<\/span><span data-contrast=\"none\"> The prosecution was supported by HSE enforcement lawyer Nathan Cook.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cThis incident could and should have been easily avoided.
